This notice describes how Proteus Aquatics uses information submitted through the Private Performance application on this website.
Information collected
The application asks for your name, email address, phone number, current membership status, coaching interest, training goals, training frequency, desired start timeframe, and investment readiness. You may provide additional notes voluntarily. Please do not include sensitive medical or financial account details in the application.
How we use it
We use application information to review suitability for our private coaching programs, contact you about the application, schedule a consultation when appropriate, and plan onboarding if you enroll. Submitting an application does not create a coaching agreement or payment obligation.
Processing and access
Applications submitted on this site are processed through Netlify Forms and can be viewed by the people responsible for managing the Proteus Aquatics website and applications. Netlify's handling of data is subject to its own terms and privacy practices. We do not use the application itself to collect payment information.
Your questions and requests
To ask about the information you submitted or request its deletion, contact performancebyproteus@gmail.com. We will review and respond to reasonable requests in accordance with applicable requirements.
